Envision Unlimited is committed to providing meaningful opportunities for greater independence and community integration for all clients, regardless of their cognitive and physical challenges. The Envision Unlimited Host Family Program allows persons with intellectual and developmental disabilities to live in their community in the private home of a family. This option allows the individuals to become more self-reliant and offers greater choice.
Envision Unlimited is looking for families who are interested in helping grow the Host Family Program by sharing their lives with persons with intellectual and developmental disabilities. The family may own or rent their home and will work with Envision Unlimited to arrange for necessary services. A licensing and support process is created for each family to be sure the program meets the needs of every individual involved.
The Envision Unlimited Host Family Program is centered around the belief that our clients are functioning and contributing members of their communities. They have much to offer interpersonally and in service to others. This program gives them the foundation from which they can grow and succeed in making their life goals a reality.
THE HOST FAMILY / ADULT FOSTER CARE PROGRAM PROVIDES:
An Individual Service Plan tailored to the developmental needs of the client placed in the home; home licensing through the Illinois Department of Human Services; living opportunity for 2 persons with developmental disabilities, for a maximum of 6 individuals living in the home; and staff visits several times each month to ensure that the clients and host family are effectively dealing with any issues or concerns, and that clients are receiving a high standard of care. The model ensures that every Envision Unlimited client continues to set consistent, important personal and professional goals, with the caring support of Envision staff and their Host Family.
